5th Edition
You force one creature you can see within range to make a Wisdom Saving Throw. On a failure, you delete 1d4 powers or spells from its mind permanently. You choose the level of each spell or power, and the DM randomly determines which of the target's spells or powers are actually deleted. If powers are deleted, Hypnosis or Reality Revision can be used to restore the lost powers. If spells are deleted, Greater Restoration or Wish can be used to restore the lost spells. Regardless of what is lost, the recovery must be performed within 1 week of losing the spells or powers. If a week passes, the spells or powers are lost permanently.   A creature that prepares spells or powers, such as a wizard or cleric, that is targeted by apopsi loses access to those spells they currently have prepared and is unable to prepare them again unless the spells or powers are restored, or if they re-learn the spell or power from elsewhere.
 

This is a rare option

This option requires special permission to take. In the case of spells, psionic powers, and similar options, this option must be found in the world and cannot be taken upon level-up.
