The first "accidental" casting should supposedly have happened during a battle. Firebringer Rheyzor Greyskull should have been surprised by a group of Wilderbeast that allegedly had snuck up upon him. In response, he cast his new experimental spell Implosion, an 11th level evocation spell that forced a creature to implode on itself. The result was successful....and gory.
