Safety Incident-Hatchery 3B

An internal report from Blackwell Fisheries concerning a rash of injuries at one of the hatchery sites. Several employees reported safety concerns with the site, then left the company according to the Chief of Public Outreach. The employees never filed for worker compensation, so the incidents were left unaddressed until forgotten, and a new second shift crew was hired.

  Incident reported by: Tyler Winston   Date: 4/11   Description of Incident: Came in and found Rob on floor, unconscious. Lots of blood on floor, appears he hit his head. Floor was wet, so it looks like he slipped. Was wearing all appropriate PPE. Sent Rob to infirm to see if concussion.

  Incident reported by: Burt Womitch   Date: 4-11   Description of Incident: I cut my hand while I was cleaning up the floor. There was a flange of bent metal on the tank. I bandaged it up using the first aid kit.

  Incident reported by: Burt Womitch   Date: 4-11   Description of Incident: I slipped and twisted my ankle on the walkway between 3B-1 and 3B-2. I have reported on the insufficient non-slip surface before. Please have management approve my standing request for Facilities to fix the walkways.   Tyler Winston also filed a Facilities request to fix the light sensors, noting that the automatic lights would shut off intermittently while the crew was working.   Robert Aaronson did not return, after being moved from the infirmary to the hospital in Sewardsville.

  Incident reported by: Rona Barr   Date: Apr 12   Description of Incident: Fish bite while clearing filter pipe. Bit through glove.   Rona also reported a possible intruder in the tank on April 12, noting that the cohort of catfish in that tank should only be up to a foot long, with insufficient teeth to bite through her neoprene gloves.

  Incident reported by: Tyler Winston   Date: 4/13   Description of Incident: Fish were agitated, and jumping from tank. I was hit by a few, got some cuts on face from barbs. Burt fell in tank, his ankle was still injured, couldn't stand back up. Shouted that something pulled him. Could not find him, lights keep going out, called sheriff. Rona and I investigating tanks now, starting search until sheriff arrives.   I could not find any more incidents logged by any of the crew, or logs of a search party from the sheriff's department for this date. A new second shift was rotated in by April 18th.
