Vorpal Garrotte

When not in use, this item is indistinguishable from an ornate belt.
When wielding this item, you can use the Attack action to make a special melee attack, a garrotte. If you’re able to make multiple attacks with the Attack action, this attack replaces one of them.
The target of your garrotte must be no more than one size larger than you and must be within your reach. You try to seize the target by making a garrotte check instead of an attack roll: a Dexterity (Acrobatics) check contested by the target’s Strength (Athletics) or Dexterity (Acrobatics) check (the target chooses the ability to use). You succeed automatically if the target is incapacitated. If you are hidden from the creature, or if the creature is has the paralyzed or stunned conditions, it makes the check with disadvantage. If the creature can see you and is not under the paralyzed or stunned conditions, it has advantage on the checks. If you succeed, you subject the target to the restrained condition and is considered to be suffocating. While garrotting the target creature, attacks against you have advantage and you have disadvantage on dexterity saving throws unless you release the target. You also have a movement speed of 0 while garrotting a creature, unless you take the Beheading action as described below You can release the target whenever you like (no action required).
Escaping a Garrotte. A garrotted creature can use its action to escape. To do so, it must succeed on a Strength (Athletics) or Dexterity (Acrobatics) check contested by your Strength (Athletics) check
Beheading. If a creature is restrained by you as a result of a garrotte attack, you can use an action to squeeze the garrotte tighter. To do so, you must succeed on a Strength (Athletics) contested by the target's Strength (Athletics) check. On a success, you deal 1d4 + your Strength modifier slashing damage to the creature. When roll a 20 on this Strength (athletics) check, you cut off one of the creature's heads. The creature dies if it can't survive without the lost head. A creature is immune to this effect if it is immune to slashing damage, doesn't have or need a head, has legendary actions, or the GM decides that the creature is too big for its head to be cut off with this weapon. Such a creature instead takes an extra 6d8 slashing damage from the hit. You can move up to half you movement speed as part of this action, unless the target is two or more sizes smaller than you, when you can move all of you movement speed.
