Makkaneth
When the spark screamed itself into existence, the abyss lashed out and shattered reality. The concepts of pain and fear came into being at the beginning of everything, and have persisted to this day. These energies coagulated into two different primordials. Shayd became the embodiment of fear, and Makkaneth the embodiment of pain.
Makkaneth has the ability to turn pain into power. Throughout much of the first iteration of the universe, most entities had no "physical" bodies capable of feeling pain as mortals know it. Pain comes in many shapes and forms. The pain of loss. The pain of failure. The pain of embarrassment, even the pain of boredom are all things Makkaneth can amplify and feast upon.
As a being who actively went out of their way to be hostile to the Pantheon, Makkaneth was banished from Zambaleth using bizarre magics. After traveling the cosmos for some time, Makkaneth learned that other entities of his caliber have built entire Paradises for themselves.
He envisioned a utopia of wretchedity and suffering, and began collecting the vast amounts of energy that would be required to spawn a realm. He dedicated his entire being to this goal, but still lacked the ability to create machinisms of pain. If he were to create a realm, it would be devoid of anything of actual use. Makkaneth continues the pursuit of their goals, but the solutions evaded him until he came into contact with the patron of fear, Shayd.
The two realized they empowered each other simply by remaining in each other's presence. During the exploration of this phenomenon they discovered they had the exact same origin. The fact that they were not only friends but even siblings eternally united them in their goals. Pain causes fear. And fear is a type of pain. Together they create a realm of sheer suffering and populate it with entities capable and driven to have hope. They gain power from the pain and fear generated by crushing those hopes. Over and over in an endless feast.
Entities that are affected by these deities go insane rapidly unless they were designed to live in the realm. The denizens are durable enough to withstand said feasts, but most other entities from outside the realm fall victim to immense and debilitating hallucinations and emotional outbursts. Eventually the entity will become a husk of itself and repurposed to be a denizen of the realm. Whatever residual sentience remains of the original entity suffers worse than the being it has become.
Makkaneth, Shayd, and the realm of suffering are considered the epitome of evil by almost all entities.

Shayd - this patron of fear is probably Makkaneth's only true friend. Their abilities compound on each other to make them incredibly powerful while on each other's presence.
Pnumesthersept - the fae deity Pnumesthersept was drawn to the strange and unique energies that Shayd and Makkaneth were tinkering with, and found her way to their realm. She was met with two overwhelming sensations. Pain, fear, her very being writhed while her mind swamed with ideas that made no sense. What did she have to fear? What was pain to a being of her magnitude? She has proven time and time again that northing can hold her back, not even as a speedbump. She adapted rapidly to fear and pain, integrating them into her being. The fear and pain are still things she feels, but it's less like a bed of nails and more like a warm blanket. The reassurance that even raw fear and pain are things she has nothing to worry about. She uses this newfound knowledge and huge jump in power to assist Shayd and Makkaneth with the stabilization of their realm, and took the opportunity to experiment with the progenation of entire races. She created beings capable of hopes and dreams, specifically so that those hopes and dreams can be crushed. Unlike entities born outside the realm of suffering, these creatures can withstand in immense amount of pain and fear without being destroyed, while still feeling pain and fear in it's entirety. A resounding success, the skills she learns here are an integral facet of the complexities to create life.
Zal'Thunas - the being is power incarnate, and has tried to obliterate Makkaneth on several occasions. The pain generated by Makkaneth being destroyed would rapidly reamalgamate and rebirth Makkaneth as a stronger entity. Eventually Zal'Thunas worked with the rest of the pantheon to banish Makkaneth using bizarre magics.
Vestivious - a patron of knowledge, this primordial is always interested in the less common philosophies and ideologies. Vestivious is not one to shy away from knowledge just because it "hurts". She happily listened to Makkaneth and it's version of morality, progress, survival, and death. After learning of pain, she restructured herself to be immune to it's detriments. While still painful, pain is much easier for her to surpass, and she does not suffer the debilitations that come with crippling agony.
