The Bloody Matron

Patron: The Bloody Matron, Queen of Envious Offerings

They call her the Matron of Crimson Thirst. To some, she is a goddess of ritual. To others, she is a devil in red silk. But to you, she is power—raw, vicious, and waiting to be fed. The Bloody Matron is a Void Prince who manifests when love sours and loyalty fails. She whispers promises to the overlooked, the abandoned, the betrayed—and demands blood in return.

Those who follow her path leave no betrayal unpunished and no rival unbled.

(1st Level)

Patron Feat: Crimson Channeling

You gain proficiency in Intimidation. If already proficient, double your proficiency bonus.

You may offer a creature’s spilled blood (killed within 1 minute) as a bonus action to your patron. Until the end of your next turn:

• Your next Voidblast attack deals maximum damage.
• If it kills a creature, you regain 1 Pact Die or a 1st-level spell slot (your choice).

This can be done once per short rest.

Voidblast

Your Voidblast deals psychic or necrotic damage (your choice when you first deal damage with it). It takes on a sanguine, shadowy form, lancing tendrils, red mist, or heart-thumping vibrations.

Crimson Fang

Your Voidblast lashes out with a violent, stabbing motion like a blood-formed fang.

• On hit: Target must succeed on a Constitution saving throw or begin bleeding (1d4 necrotic damage at the start of each of its turns for 1 minute). Target can repeat the save at the end of each of its turns to end the effect.

Ritual: “First Offering”

“Every mother feeds her young. Every Matron feeds on blood.”

Components: Fresh blood from a living creature (1 HP minimum), a bowl of carved bone, and your own heartbeat timed to a chant
Casting Time: 10 minutes
Effect: You summon your familiar Yara, a tiny, red-skinned creature with bulbous eyes and suction-cup fingers.

The blood in the bowl bubbles into a pulsing shape before leaping into the air and forming Yara. It immediately wants to bite something.

• You share telepathic communication (60 ft) with it.
• It levels up at class levels 1, 5, 10, and 15, gaining new powers and actions.
• This familiar links you to your patron and powers

Traits

• Blood Scent: Can detect creatures below half HP within 30 ft.
• Crimson Leech (1/day): Target hit by its bite reduces their next healing effect by half.
• Drain Offering (Action): If a creature is dying within 5 ft, Yara may extract a blood offering. Doing so empowers your next Voidblast to deal an extra 1d6 psychic damage.

New Traits

• Blood Feast: Whenever it reduces a creature to 0 HP, you gain temp HP equal to your Charisma modifier + level.
• Vampiric Mimicry (1/day): After consuming a foe’s blood, Yha can mimic one of their abilities (e.g., a spell, reaction, or movement type) for 1 hour.

Attack
Soul Sucker Bite: +6 to hit, 2d8 + 2 necrotic; DC 14 Con save or target is weakened (half weapon damage) for 1 round.

Bloodburst Echo

You learn to erupt your Voidblast across nearby foes with a visceral splash.

• When your Voidblast hits, you may cause each creature of your choice within 5 ft of the target to make a Dexterity saving throw or take half the Voidblast’s damage (no added effects).
• You can use this feature a number of times equal to your proficiency bonus per long rest.

(9th Level)

Ritual: “Cup of Envy”

“The world owes you. Make it bleed for what it has.”

Components: The name of someone you envy, written in blood on a page made of skin (animal or humanoid a silver cup cracked by rage
Casting Time: 10 minutes, must be performed during a moment of strong emotion (jealousy, betrayal, passion)
Effect: The Bloody Matron answers through a crimson vision: your blood boils in patterns only you can read. She may grant:

• A target's weakness
• A vision of betrayal
• A ritual to empower a deadly strike

But her advice is always laced with the demand: “Bring me another name.”

Stage III: Yara-ma-yha

Large Aberration

New Traits

• Bloody Bloom Aura (10 ft): Hostile creatures below half HP must make a Wisdom save (DC = your spell DC) or be Frightened.
• Blood Transfer: As a bonus action, you may use Yha-Who to transfer HP from one creature to another (up to 20 HP).

Veinrend

Your Voidblast now digs deep into the target’s life essence.

• On hit: Target’s max HP is reduced by the psychic/necrotic damage dealt until the end of your next long rest. Multiple hits stack.
• In addition, if a creature affected by this Voidblast is reduced to 0 HP, you or your familiar regain temporary hit points equal to your Warlock level.

(15th Level)

Stage IV: Yara-ma-yha-who

Legendary Traits

• Blood Godling: When it kills a creature, regain one expended spell slot or Pact Die.
• Feast of Envy (1/day): Choose one creature within 60 ft who has more HP than you. Equalize your HP totals.
• Red Throne (1/day): For 1 minute, all enemies within 30 ft bleed uncontrollably (1d8/turn, Con save ends). You are immune to critical hits during this time.

Red Bloom

Your Voidblast can erupt in a 20-foot crimson bloom.

• As an action, choose a point you can see within 60 ft. A Crimson Bloom erupts in a 20-ft-radius sphere.
• All creatures in the area must make a Constitution saving throw or take 6d10 necrotic or psychic damage (your choice), or half on a success.
• Creatures that fail the save are also bleeding (as Crimson Fang) for 1 minute.
• You may use this feature once per long rest, or expend a Mythic Deed slot to use it again.
