Ozzurog is a vast, pulsing mass of viscous black slime, thick as tar and shot through with glistening streaks of purple and green iridescence. It has no skeleton, no central shape, only an endless, ever-shifting mound of slick, pulsating ooze. From this sludge sprout dozens of hundreds of eyes, some human, some bestial, some unmistakably alien. It may look like an ooze on the surface, but Ozzurog is more than just a pile of goo, corrupted by the Far Realms. It emits no true sound, but when nearby, one may hear the subtle squelch of unseen movement and the soft rustle of lidless gazes shifting in unison.

Traits
Legendary Resistance (3/Day). If Ozzurog fails a saving throw, it can choose to succeed instead.
Amorphous Form. Ozzurog can move through a space as narrow as 1 inch without squeezing.
All-Seeing Eyes. Ozzurog cannot be surprised or blinded. It sees everything within 300 feet unless magically obscured by divine intervention or a wish.
Eldritch Corrosion. A creature that ends its turn grappling, grappled by, or inside Ozzurog takes 21 (6d6) 6d6 acid damage and must succeed on a DC 21 Wisdom saving throw or gain a level of madness (DMG p. 258) and one permanent eyeball appears on their body, controlled by Ozzurog.
Actions
Multiattack. Ozzurog makes three attacks: one with Tendril Lash and two with Mindpierce Gaze.
Tendril Lash. Melee Weapon Attack: +13 1d20+13 to hit, reach 20 ft., one target. Hit: 23 (4d8+5) 4d8+5 bludgeoning damage plus 13 (3d8) 3d8 acid damage, and the target is grappled (escape DC 20). Until the grapple ends, the target is restrained and takes 13 (3d8) 3d8 psychic damage at the start of each of its turns.
Mindpierce Gaze. Ranged Spell Attack: +13 1d20+13 to hit, range 120 ft., one creature. Hit: 27 (6d8) 6d8 psychic damage, and the target must succeed on a DC 21 Wisdom saving throw or be stunned until the end of its next turn.
Eye Plague (Recharge 5–6). Ozzurog targets up to 3 creatures it can see within 120 feet. Each target must succeed on a DC 21 Constitution saving throw or grow unnatural eyes on their skin. A greater restoration or wish removes the eyes. While afflicted:
- Ozzurog sees through the eyes (no action).
- The creature takes a -2 penalty to attack rolls and saving throws involving sight.
- The creature must repeat the save at the end of each long rest. On 3 failures, it becomes Ozzurog’s thrall (dominate monster, no concentration required).
Legendary Actions
Legendary Actions (3/round)
Ozzurog can take 3 legendary actions, choosing from the options below. Only one legendary action can be used at a time, and only at the end of another creature’s turn. Regains all legendary actions at the start of its turn.
- Slick Step. Ozzurog moves up to 30 ft. without provoking opportunity attacks.
- Tendril Swipe. Makes one Tendril Lash attack.
- Reveal the Unseen (Costs 2 Actions). All creatures of Ozzurog’s choice within 60 ft. must succeed on a DC 21 Wisdom saving throw or be blinded and frightened until the end of their next turn as their vision floods with alien truths.
- Tear the Veil (Costs 3 Actions). Ozzurog opens a rift into an alien dimension within 120 ft., summoning 1d4 Gibbering Mouthers or 1 Star Spawn Hulk (DM's choice). The rift lasts 1 minute or until Ozzurog dies.

General Physical Condition
Ozzurog speaks not with a voice, but through dreams, reflections, and vision loss. When it does speak directly, it does so through a warlock or a temporary form.
Being near Ozzurog feels like being watched by something too vast to comprehend, not malevolent at first, just... aware. The air around it is heavy with dread and the smell of wet stone and decay. Mirrors warp. Eyes itch. Perception distorts. Its presence overrides reality, tilting the laws of perception. In Ozzurog’s gaze, lies peel away, time loops, and the act of seeing becomes dangerous.
Body Features
Each eye blinks independently, swivels, narrows, or dilates in silence.
Tendrils emerge like grasping hands, ropes of sticky goo tipped with twitching eye stalks or whispering mouths.
Wherever it moves, it leaves a smear of slime that births more eyes if not cleansed or consecrated.

All-Seeing Eyes
Endless Vision: Every eye on its body sees across dimensions, past, present, and possible futures. It can see through walls, into souls, and between worlds.
Truth Gaze: Anyone who makes eye contact with one of its eyes is paralysed by overwhelming truths, memories not their own, cosmic visions, or unbearable self-awareness.
Eye Plague: It can plant eyes on living flesh. These parasite-eyes sprout on victims’ skin, allowing Ozzurog to watch through them and slowly drive them mad with shared sight.
Goo Form & Tendril Flesh
Amorphous Form: It has no bones, no stable body. It can slither under doors, squeeze through keyholes, or balloon to fill entire chambers with its pulsing mass.
Corrupting Touch: Its goo melts flesh, but not with heat; it rewrites organic matter into more of itself. Those touched too long become eyestalk-riddled thralls.
Tendril Puppeteer: Its gooey tendrils can pierce into nervous systems to control bodies like marionettes, living or dead.
Sight Warping
Perception Twist: It can distort reality through visual manipulation. Victims see illusions, shifting surroundings, or even believe their own bodies are wrong (missing limbs, reversed faces, etc.).
Eye of Revelation: It can reveal a truth so profound or alien that it shatters one's mind, for example, showing a mortal what the world looks like through the eyes of a god, a corpse, or a swarm.
Veil Tear: It can “see” where dimensions are thin and rip them open, stepping into other planes or pulling things through.
Soul Sight & Possession
Soul Mirror: Ozzurog can peer into someone's soul and speak in the voice of their inner thoughts, dreams, or regrets. It uses this to manipulate or seduce.
Host Binding: It can hide within a human, speaking through them, enhancing their vision, or slowly taking them over.
Memory Feeding: It consumes memories through eye contact or goo infiltration. Victims lose their past, growing confused, hollow... and eventually obedient.
