If the target fails their saving throw, at the beginning of their turn for the duration of the spell, they take 4d6 psychic damage, plus an additional 4d6 psychic damage if the target is concentrating on a spell. The target may repeat the saving throw at the end of each of their turns, ending the effect on themselves on a success.
