The Abyssal Plague
The abyssal plague is a disease that originates from a dead universe, one where the Voidharrow – their version of the abyss – succeeded in destroying all of reality. Semi-sentient, it used the chained god Tharizdun, who is a prisoner in its world, to enter other worlds and infest them.
Those infected by the disease slowly develop oozing sores, blisters, and growths that appear as crimson crystal laced with veins of silver and flecks of gold. The crystal can be in either liquid or solid form. Most are quickly driven mad as the disease ravages the mind while the body slowly turms into that of a Plague Demon.
Contracting the Disease
There are several ways for the abyssal plague to spread: through consuming the Voidharrow itself, coming into contact with it or with the blood of another infected creature, or by the attack and death throes of a plague demon. The disease also spreads through food and drink contaminated by diseased flesh.
Disease Progression
There are 3 main stages to the abyssal plague, each progressively worst than the last. The disease progresses on a day to day bases. At the end of a long rest, an infected creatures makes a Constitution saving throw, the DC of which is equal to 14 + the stage of the disease.
On a success, decrease the stage of the disease by 1. On a failed save, there is no change. If however the creature rolls a number equal to or lower than 8 + the stage of the disease on their save, increase the stage of the disease by 1.

The creature exhibits sores and growths across parts of the body and loses the use of one hit die |
Stage 2: |
The creature has sores and growths over half their body and loses the use of two hit dice.
The creature also takes a -2 penalty to their AC.
The creature has disadvantage on Dexterity saving throws
The creature's movement speed is halved. |
Stave 3: |
Sores and growths cover most of the creature's body.
The creature's hit point maximum decreases by 3d6 every 24 hours.
The creature becomes increasingly disoriented and chaotic as the demonic nature of the disease takes hold. |
Stage 4: |
The creature dies. If the creature was a beast, dragon, giant, humanoid, or monstrosity, The creature immediatley rises up as a plague demon. |
Curing the Disease
Being somewhat divine in nature, the disease can only be cured by a spell cast at 5th-level or higher. An Elixir of Health, a Potion of Vitality, and other magical items can also cure or prevent the plague from taking root.
Once a creature has turned into a plague demon however, there is no method of turning them back short of a wish spell. A dead plague demon can't be resurrected or reincarnated, as its soul is currently part of the Voidharrow.

Aggressive. As a bonus action, the plague demon can move up to its speed toward a hostile creature that it can see.
Madden Fury. The plague demon deals an extra 3 weapon damage when it hits with a melee attack (included in the attacks).
Plague Riddled. Whenever the plague demon deals necrotic damage to a creature with a melee attack using its natural weapons, the target must succeed on a DC 12 Constitution saving throw or be infected with stage 1 of the abyssal plague.
Actions
Multiattack. The plague demon makes one bite attack and one claw attack.
Bite. Melee Weapon Attack: +3 to hit, reach 5ft., one target. Hit 7 (1d6 + 4) piercing damage + 2 (1d4) necrotic damage.
Claw. Melee Weapon Attack: +3 to hit, reach 5ft., one target. Hit 11 (2d6 + 4) slashing damage.
