With an Action you open up a portal to spot on the same plane as you are now. The DM will roll from the teleportation outcome table based on how familiar you are to the location what will happen.

Time Cost: 5 Crafting Checks
Crafting check: Tinkerer's Tools DC16 (Long rest DC21)
