Duke of Vann'fjell and Elvegard is a combination of the titles Lord of Vann'fjell and Duke of Elvegard. Since both of these titles are given to the King of Vann'ufalme, and since Vann'fjell is located within Elvegard, they are almost always abbreviated in this manner.
