Darogan
It was in the ancient ruins of the Feywood, before a strange and transformed pool, that the heroes encountered someone alien to them. As each member of the party was pulled into a strange void, they witnessed elements of their past rush up before them in a chaotic mixture while some imperceivable being scrutinised their minds.
After this mysterious intelligence was finished with whatever sinister work it operated on the heroes, they were discarded back into the real world with searing brands on their shoulders - the scarred image of an eye.
Later, as the heroes reached the dying Trin who they attempted to save from Mayor Trigby's efforts, they would learn more about this strange being. He is named Darogan - a mysterious warlord who is responsible for shattering the barriers between worlds that had prevented all mortal knowledge of a multiverse at all.
Trin explained that Darogan has been warring across the multiverse in search of her. He undoubtably sought to claim the treasure she defended with her life - the Codex Multiversalis, an artifact from the dawn of creation that protects the multiverse and enables its wielder to traverse it effortlessly. Trin told the party that Darogan was seeking all of the Codices for some purpose, but that whatever that purpose was would not be good. For whoever controlled all of the ancient books of creation would be themselves a god.
In her dying breath, Trin sacrificed herself to give the Codex Multiversalis to the heroes; the act of which killed her. The party had been left with questions and little answers about a foe whose eye is literally upon them... and an object this foe craved above all else. While Trin had revealed the location of more of the Codicies to the party, with her death the party had inherited a burden no mortal had possessed before - to prevent a mysterious tyrant from conquering a multiverse they are only just discovering.
One thing is clear: the heroes have a great deal to learn about the task that befalls them... and the enemy whose eye was on them.
