A creature must either ingest or be injected by a poison to gain this effect  
  • A poisoned creature has disadvantage on attack rolls and ability checks.
  A creature can make a constitution save equal or higher than the poison Save DC, if they fail they can not resist the effect till the poisoned condition ends. It can only be relived by an antidote or healing effect that can cleanse poison   Any additional effects are reliant on which poison is used
