Drown
You speak an unholy word and vow to make a creature suffer. The target makes a Constitution saving throw, having their lungs filled with water on a failure. The creature can hold its breath to avoid drowning. The target can make a new saving throw at the end of each other of their turns to expel the water. Another creature within 5 feet of the target can use their action to attempt a Medicine check with a DC equal to your spell save DC to end this spell on the target, forcing the water from their lungs.
While holding their breath with water-filled lungs, a creature cannot talk or cast spells with verbal components, and has disadvantage on attack rolls, and ability checks.
A creature with its lungs filled with water in this way can stay conscious a number of rounds equal to 1 + their Constitution modifier (minimum 1 round).
This is a rare option
This option requires special permission to take. In the case of spells, psionic powers, and similar options, this option must be found in the world and cannot be taken upon level-up.
