The caster temporarily strains the subject's muscles in a very specific way. The subject feels a sharp pain whenever she makes an attack. All her attack rolls have a -2 circumstance penalty for every four caster levels (maximum penalty -10).
 
5th Edition
You attempt to strain the muscles of a creature you can see within range in a very specific way. The target must make a Constitution saving throw, feeling a sharp pain whenever he or she makes an attack on a failure. While feeling this pain, the target has Disadvantage on attack rolls.   This spell has no effect on Undead creatures.
