Tiamat is a level 24 sorcerer. Her spellcasting ability uses her Charisma score (Spell Save 27, +17 to hit with spell attack). She can cast the following spells without needing material costs:

  • Cantrips (at will): all cantrips
  • Level 1 (at will): bane, chromatic orb, command, detect magic, identify, inflict wounds, shield
  • Level 2 (at will): blindness/deafness, knock, misty step, ray of enfeeblement, suggestion
  • Level 3 (at will): bestow curse, dispel magic, incite greed, remove curse, spirit guardians, vampiric touch
  • Level 4 (4 slots): banishment, blight, death ward, elemental bane, Leomund's secret chest
  • Level 5 (3 slots): antilife shell, circle of power, contagion, legend lore, scrying
  • Level 6 (3 slots): circle of death, disintegrate, mass suggestion, scatter
  • Level 7 (3 slots): divine word, finger of death, plane shift (self only), prismatic spray
  • Level 8 (2 slots): earthquake, feeblemind, illusory dragon
  • Level 9 (2 slots): gate, power word kill, wish

1/day:

Tiamat can divinely cast Time Stop once an encounter. Beware! Instead of Time Stop ending whenever Tiamat affects a creature other than herself with an action or ability, an effected creature instead must roll either a Strength or Charisma saving throw of 25 or greater to break free of the spell, with each consecutive action or ability affecting the creature lowering the threshold by 4. Clerics of good-aligned deities can use their Reaction and Divine Intervention to prevent Tiamat from using this feature for 24 hours.

3/day:

Summon Abishai: Tiamat summons her personal devils, the abishai, from the Nine Hells to assist her in combat. Once Tiamat casts the spell, 5 summoning circles appear on the ground. At the beginning of Tiamat's next turn, her abishai will manifest from these circles, with one circle summoning 1 red abishai, one circle 2 blue abishai, one circle 2 black abishai, one circle 3 green abishai, and one circle 3 white abishai. If a creature or creatures want to prevent a summoning, that creature(s) must enter one of these circles and spend their turn in that circle, while succeeding a Charisma roll with DC 20 to will the summoning from succeeding. The more creatures in a circle, the lower the Charisma DC becomes.


Waterbreathing: Tiamat can indefinitely breathe underwater (though as a divine being she doesn't need to breathe). Tiamat may also freely use her breath weapons, cast spells and other abilities while submerged.

Anchored Soul: Tiamat's magical and divine nature makes it difficult to move her magically to other planes of existence without her willing it. Any attempt to send Tiamat to another location on the same plane or to a different plane must succeed a spell save DC of 28 or immediately fail.

Immutable Form: Tiamat's divine nature and indomitable will prevents her body from being altered into another form that she doesn't will it to.

Legendary Resistance (5x a Day): if Tiamat fails a saving throw, she may choose to succeed instead.

Multiple Heads: Tiamat has five heads, each capable of doing different tasks at the same time.

  • Tiamat has a number of Reactions equal to the number of heads that she has, up to 5 Reactions.
  • Each head can concentrate on its own spell.
  • Each head has a saving throw against being knocked unconscious.
  • If Tiamat fails a saving throw against an effect that would stun a creature, one of her unspent legendary resistances is spent.
  • If a creature targets one head of Tiamat and deals 200 or more damage in a single attack, or attacks wielding a vorpal sword rolls a natural 20 on an attack against Tiamat, the chosen head is severed and dies. The missing head regrows after 3 rounds of combat.

Siege Monster: Tiamat deals double damage to objects and structures.

Regeneration: Tiamat regains 30 health points at the beginning of her turn.

Treasure Sense: Tiamat knows the location of all magic items and mundane collections of treasure within 5 miles of her. She has advantage on learning information about magical items when using Detect Magic.

Actions

Multiattack: Tiamat casts Frightful Presence, then makes a total of 5 bite or claw attacks (any combination of the two) and one tail stinger attack. Tiamat may substitute two bite or claw attacks for a breath weapon attack or a crush attack.

Bite: Melee Weapon Attack: +19 to hit, reach 35 ft, one target. Hit: 23 (2d12 + 10) piercing damage. If the creature bitten by Tiamat is a size Medium or smaller, the creature must succeed a Strength saving throw of 18 or become swallowed by Tiamat. If a creature starts their turn swallowed by Tiamat, they are restrained and take 4d8 acid damage. If the creature deals at least 30 damage inside of Tiamat, she regurgitates the creature out 10ft away from her body.

Claw: Melee Weapon Attack: +19 to hit, reach 30 ft, one target. Hit: 32 (4d10+10) slashing damage. If Tiamat chooses to, and the target is a creature, the target must make a Strength saving throw of 22 or higher or be thrown back 30 feet.

Dragon Breath: Tiamat chooses one of her heads to make a breath attack. Each head has a separate recharge time of 1d4 rounds. (This recharge time applies to any uses as a Legendary Action as well.)

  2. Blue Dragon Head: Electric Breath
  3. Green Dragon Head: Poison Breath
  4. Black Dragon Head: Acid Breath
  5. White Dragon Head: Ice Breath

Crush: Tiamat moves up to half of her movement speed, then attempts to crush anything beneath her. Large or smaller creatures must use their Reaction and make a Dexterity save of 18 or higher to avoid being crushed. Failure: the affected target is dealt 40 (4d12+10) bludgeoning damage, knocked prone, and stunned. Success: the target has to move up to half of their movement speed away from Tiamat and has disadvantage on their next two attacks and/or saving throws.
Huge or larger creatures: Tiamat instead body slams the target, with the defending target making a Strength saving throw instead. Failure: target is dealt 3d8+10 bludgeoning damage and is pushed back 30 feet. Success: half damage and no pushback.

Tail Stinger: +19 to hit, 25ft range, one target, 28(4d8+10) piercing damage and target must succeed a Constitution saving throw of 16 or higher or become Poisoned. While poisoned in this way, the creature is under effect by the Slow spell.

Frightful Presence: cast Fear with a DC of 28.

Change Shape: Tiamat magically transforms herself into a humanoid form or back to her true form (similar to True Polymorph). This alternate form is called the Dark Lady (refer to the Dark Lady stat block for details on her capabilities while in this alternate form). She reverts to her true form if she reaches 0 health points. Any equipment Tiamat is wearing or carrying is absorbed into her body or borne by the new form (Tiamat's choice).

Legendary Actions

Tiamat can make one legendary action after an enemy's turn. Tiamat has a number of legendary actions equal to the number of enemies she is facing, up to a total of 8 legendary actions. Tiamat regains all uses of her legendary actions at the beginning of her turn.

Detect: Tiamat makes a Wisdom (Perception) check.

Pounce: Tiamat moves half of her movement speed and uses either a Bite or Claw Attack.

Infernal Wings (Costs 2 Legendary Actions): Tiamat raises her wings and beats them with great might. Creatures of size Large or smaller within 45 feet of Tiamat must succeed in a Strength or Dexterity save of 19 or higher or be dealt 2d8 force damage, pushed back 30 feet, and knocked prone. On a successful save, creatures take half damage and remain where they are positioned. (Tiamat can use this Action while hovering.) Then, if Tiamat is on the ground, she may choose to take flight, moving up to half of her flight movement speed.

Frightful Presence: cast Fear with success on saving throw 25 or higher.

Each head of Tiamat has a unique Legendary Action it alone can use. If the head is severed, Tiamat cannot make that Action.

Gaze of the Nine Hells (Red Dragon Head): The eyes of Tiamat's red dragon head glow with an unworldly fire. All creatures in a 30ft sphere within 45ft of Tiamat wearing or holding metal are effected by the Heat Metal spell cast at 4th level (4d8 fire damage). Conversely, if a creature within this sphere is not holding or wearing metal but is holding or wearing something combustible, Tiamat chooses one item on that creature and it bursts into flame, dealing 3d8 fire damage to the creature wearing or holding it. That creature must spend their next Action or Reaction snuffing the flame out or the item burns up and is destroyed. Magical items targeted this way

(Blue Dragon Head):

Infernal Tongue (Green Dragon Head): The tongue of the green dragon head extends and stretches out, grappling a target within 120 feet. The target must succeed a Strength saving throw of 19 or higher to resist being pulled towards Tiamat. Failure: the target is grappled, takes 2d8 poison damage, and is pulled within 15 feet of Tiamat. Once the target is within 15ft of Tiamat, she releases the grapple and the tongue retracts back into her mouth. Success: the target is grappled, takes 2d8 poison damage, but is not pulled towards Tiamat. The grappled target may either attempt to break the grapple on their turn by either another Strength saving throw of 19 or dealing damage to the tongue up to or exceeding 30 points. Another creature may also attempt to break the grapple by dealing 30 or more points of damage or making a Strength attack roll.

Soul Consumption (Black Dragon Head): If a creature is within 10ft of Tiamat, her black dragon head targets one creature and attempts to siphon the creature's essence from its body. Target creature must succeed a Constitution or Charisma saving throw of 26 or higher or have their Constitution or Charisma reduced by 2d8, whichever score was used to resist Tiamat. On a successful save, the creature instead takes 2d6 necrotic damage.

Bone-Chilling Mist (White Dragon Head): The white dragon head emits a thick, cold fog that heavily obscures a 400 feet area around Tiamat. Creatures other than Tiamat take 2d10 cold damage when the mist is created, and take an additional 1d10 cold damage for each turn they stay within the mist. Then, Tiamat can use her Detect ability and move up to half of her movement speed on the ground.

An enormous creature of stature and presence, Tiamat is a gargantuan reptilian being with five, serpentine heads and necks sprouting from her torso. Each head and neck resembles one of the chromatic dragons, usually a black, blue, green, red, and white elder wyrm in appearance and size, each neck roughly 80ft in diameter.

Her torso is like a mountainside, about 300 ft across from shoulder to shoulder, covered with small, red and black scales the size of a man's fist on her sides, limbs, and back, and large, pale, segmented, plate-like scales covering her belly up into each neck and into her tail.

Each limb of Tiamat ripples with muscle ready to pounce, with three clawed fingers and opposable thumbs for her arms and four clawed toes on her legs.

Her wings are massive, with a wingspan reaching (insert # here), covered with fine, black scales on the back side and red scales on the inside. Though muscular, these wings are not fully able to bear Tiamat's weight during sustained flight, relying on her innate magic and divinity for prolonged use.

Tiamat's tail is unusually long, making up almost half of her total length. Covered in small, black scales, her tail ends in a wicked, barbed stinger, similar to that of a scorpion.
